Screen curtains draped over a hook on the wall - not in use. Corrective Action(s): Ensure if back door is left open that the screen is in use. If not in use keep back door closed. Ensure screen has a complete seal on the entire door when in use to prevent pest entry by crawling or flying. Repeat violation. Violation Score: 3

Test strips present for monitoring. Bleach sanitizer present in spray bottle for sanitizing food contact surfaces. *Ensure sanitizer bottle is replace - spray not consistent No signs of pest activity noted during inspection General sanitation of premises is excellent Required repairs: 1. 2 door undercounter freezer - currently out of order - waiting on service person 2. Small leak under dishwasher drain basket - waiting on service person to come back
