Observation (CORRECTED DURING INSPECTION): 1) cutting board had dark sports and streaks over it. Operator stated it was only washed with soap and water. It was too big to fit inside the wash sink.
corrective action(s): As the dark sports were too big to clean properly, discard the cutting board and replace. Ensure cutting board is properly sanitized to prevent pathogen growth.
2) Dough mixer handle was found with pieces of dough stuck to it. Operator stated it was used the night before.
Corrective action(s): Operator was asked to wash and sanitize the handle during inspection. Clean and sanitize the dough mixer parts after every use to prevent pathogen growth.

Observation (CORRECTED DURING INSPECTION): Staff on-duty was wiping hands on her clothes during pizza preparation. Multiple incidents were observed.

Observation (CORRECTED DURING INSPECTION): 1) Anchovies were found stored in the original metal can and in single-use aluminum container inside the cooler.
Corrective action(s): Anchovies were discarded during inspection. Metal cans and aluminum containers can be corroded and contaminate food.
2) Cut bell peppers were stored in cardboard boxes inside the walk-in cooler.
Correction action(s): Store cut vegetables in smooth, non-absorbent, and easily cleanable containers to prevent contamination.

Observation (CORRECTED DURING INSPECTION): A Ziploc bag was washed and being reused. Operator stated it was used to store pepperoni and washed with soap and water.
Corrective action(s): Ziploc bags cannot be properly cleaned & sanitized. Do not reuse them. Bag was discarded.

Observation: Staff on-duty was alone and had no valid Foodsafe Level 1.

Comments:
-Walk-in cooler: 3-4C.
-Prep cooler (top): 4C (bottom): 1C.
-Small chest freezer: -18C.
-Large chest freezer: -18C.
-Glass beverage cooler was not storing potentially hazardous foods. Only carbonated beverages were stored.
-No hot-holding units were observed.
-Two-compartment sink was used and bleach solution was prepared at 100 ppm.
-Sanitizer bucket had 100ppm bleach solution.
-Handsink had hot & cold running water, liquid handsoap and single-use paper towels.
-No signs of pests were observed.
-Foods were cooled properly and covered with metal pans.
-Thermometers were work and available in al cold-holding units. Recommend to obtain a portable thermometer to ensure electronic thermometer attachment on the prep cooler is working.
-Permit was posted conspicuously.
-Recommend to keep a temperature log for cold & hot holding units.
-Please consult health inspector about the change of equipment (dishwashing sink).
