Observation (CORRECTED DURING INSPECTION): QUATS sanitizer spray bottles (x3) measured at 0-50 ppm QUATS concentration during time of inspection. Operator mentioned that the QUATS solution in the spray bottles is changed every night. Dispenser at back/mop sink area measured at 0 ppm during time of inspection. Dispenser nozzle observed blocked and was fixed during time of inspection to dispense 200 ppm QUATS concentration. Solution in all spray bottles was replaced during inspection.
Corrective Action(s): Ensure QUATS sanitizer concentration is checked frequently with available test strips (as per approved sanitation plan) to ensure sanitizer is dispensed at 200 ppm QUATS concentration to effectively remove potential pathogens from food contact surfaces and to prevent foodborne illness.

Observation (CORRECTED DURING INSPECTION): Bagged fish observed submerged in still water in prep sink. Staff mentioned the fish was thawing in the water and turned the tap back on.
Corrective Action(s): Ensure all frozen potentially hazardous foods are thawed by using one of the following proper thawing techniques:
- Under running water at all times
- In a microwave as part of the cooking process
- In a walk-in cooler (stored at 4'C)

Observation (CORRECTED DURING INSPECTION): Back door observed propped open upon entry to inspection. Staff closed the door during time of inspection.
Corrective Action(s): Ensure all pest point of entry areas are mitigated by keeping windows and doors closed at all times (other than times of receiving). If the door is to be opened for fresh air, consider installing a screen door on the back door to prevent pests from entering the premises.

2. Sanitation:
- General sanitation of premises satisfactory (except for above noted violations)
- Hand washing stations throughout premises supplied with liquid soap, paper towel, hot and cold running water
- 2-compartment sink available with drain plugs
- QUATS dispenser at 2-compartment sink measured at 200 ppm QUATS concentration
- High-temperature dishwasher reached 75.0'C at plate surface (final rinse temperature exceeds minimum of 71'C)
- In-use rice scoop changed every hour (sign available next to scoop as a reminder)

3. Pests:
- No signs of pests observed during inspection
- Sticky yellow fly trap in place in prep area

4. Storage:
- All food stored at least 6 inches off the floor
- All food observed covered
- Time stamps available on food containers (first-in-first-out method in use)

5. Administration:
- FOOD SAFE Level 1 trained staff available on premises
- Operating permit visible during time of inspection
