Observation (CORRECTED DURING INSPECTION): Cold potentially hazardous food (bean sprouts, egg rolls, and deep fried meat products) were above 4 degrees C (between 6.5 to 6.7 degrees C) inside the walk-in-cooler.
Corrective Action(s): All cold potentially hazardous food products were moved to another cooler at or below 4 degrees C. Frozen food was transferred into two chest freezers since the walk-in-cooler was above 4 degrees C at the time of inspection.

Observation: Walls behind the handsink in the kitchen and near the 3-compartment sink contained food debris over them and require cleaning.
Corrective Action(s): Either clean or repaint the walls near the 3-compartment sink and near the handsink to remove the food debris; Correct by January 25, 2019.

Observation: 1. Seals and handle of the walk-in-cooler were taped.
2. Empty cartons were being used to line a shelf on which pots and pans were being stored.
Corrective Action(s): 1. Remove the tape as it is not made of easy to clean durable materials; Correct today. Replace the seals if they are broken or are not in good working order.
2. Operator discarded the cartons and placed the pots and pans in the dishwashing sink. Make sure all construction materials are easy to clean, impermeable to moisture, non-toxic, and durable. Do not use empty food cartons to line surfaces. [Corrected]

Observation: Walk-in-cooler was above 4 degrees C (at approximately 6.8-6.9 degrees C) at the time of inspection. Operator called a technician to repair it.
Corrective Action(s): Adjust/repair the walk-in-cooler so that it maintains a temperature at or below 4 degrees C (40 degrees F); Correct by January 17, 2019. Do not store any potentially hazardous food (raw meat, previously cooked meat, bean sprouts, sliced vegetables, tofu, etc.) inside it until it is maintained at or below 4 degrees C (40 degrees F); Effective immediately. Operator stated that they will be onsite by 1 PM tomorrow for the follow up inspection.

Handsinks in the kitchen and washroom were supplied with liquid soap, hot and cold running water, and paper towels.
Domestic cooler was at or below 4 degrees C.
Three chest freezers and freezer component of domestic cooler was at or below -18 degrees C.
Hot-held rice was at or above 60 degrees C.
100-200 ppm chlorine sanitizer was available inside a bucket. Operator placed a clean wiping cloth into the sanitizer during the inspection.
No signs of recent pest activity was present at the time of inspection.
Ventilation hood was in a clean condition.
Only take-out (single-service utensils) are permitted at this facility. Single-use take-out containers were available on-site.
Operator had a FOODSAFE Level 1 certificate that is valid until September 20, 2020.
