Observation (CORRECTED DURING INSPECTION): Surface sanitizer solution not being used in the back of house. Sanitizer out of dispenser measured 0 ppm QUATS, container of sanitizer was empty
Corrective Action(s): replaced. ensure that sanitizer solution is maintained in spray bottles at 200 ppm QUATS and frequently used around the premises for sanitizing surfaces.

-numerous flies observed in the back of house, particularly around the staff changing room
-numerous fruit flies observed in bar area broken glass bucket
-dead cockroaches found in various areas of the premises (in draft beer catch bottle at the bar, on sticky trap beside ice machine, behind slush machine, and behind the lottery POS, and in the elevator mechanical room
Corrective Action(s): clean the back of house staff change room floors thoroughly, re-install the metal cover onto the floor, clean the broken glass bar bucket on a more frequent basis, and clean the above noted areas identified to have cockroaches.

COVID controls:
Operator has posted physical distancing and entry exit signs around the facility however it was found that customers were ignoring signs and leaving from the front door as well as leaving tables and talking to servers. Advise operator to install more signs and have servers review controls and restrictions upon seating customers and to install directional arrows on floor at key areas of the facility.
