Beverage cooler and 2 shelf layers are missing and will be in place today. -Pest Control program in place -Food safe certificate, Food safety plan, sanitation plan, application for health approval submitted -Permit fee still required for $87.50 -Quat sanitizer at 200 ppm -Garbage bins available -Paper towel available at all hand sinks -Thermometers available and equipped in all coolers Health approval has been granted up submission of the required permit fee. Any changes to the service or physical premise is subject to new health approval. This does not exempt you from approvals required by other agencies -Empty cardboard boxes will be cleaned up and removed
