208 - Foods obtained from unapproved sources [s. 11]
Observation (CORRECTED DURING INSPECTION): [REPEAT VIOLATION] Frozen individually packaged raw red meat observed stored in the chest and display coolers. Some packages are observed stored in black garbage bags. No label available for meats. Operator is not able to provide invoice for meats to confirm if the meat is from an approved source during time of inspection. Risk of selling meat from unapproved source was discussed with operator. 32 packs of frozen red meat were voluntarily discarded by the operator during time of inspection.
Corrective Action(s): DO NOT OBTAIN meat from unapproved sources. All foods must be obtained from an approved supplier to prevent foodbone illness to customers. Invoices must be received upon receiving all food and kept on site for verification by the health inspector. Failure to comply in future may lead to further progressive enforcement (Corrective/Closure Order and/or ticketing).

Observation (CORRECTED DURING INSPECTION):
1. Some boxes of foods and bags of dry foods observed stored directly on the floor during inspection.

2. Operator has installed bulk food containers on a shelving unit at the front of the store without submission of updated plans to and review/approval from the district health inspector. Operator placed a sign on the bulk containers reading "not for sale" during time of inspection.
Corrective Action(s):
1. All foods must be stored at least 6 inches off the floor and covered at all times to prevent contamination.

2. Cease sales of all bulk food items until you have submitited plans to the health inspector for review AND the plans have been reviewed and approved by the health inspector. Failure to comply may lead to progressive enforcement.

Observation: Operator has started sale of bulk food items. Operator VOLUNTARILY stopped sales of bulk foods during time of inspection.
Corrective Action(s): Submit updated floor plan and food safety plan to the health inspector for all bulk food items. Plans must be reviewed and approved by the health inspector PRIOR to sale of bulk food items. Failure to comply may lead to progressive enforcement.

DO NOT sell any bulk items until approved by the health inspector. If plans are not submitted by the date below, all bulk food containers must be removed off site before the follow-up inspection. Failure to comply may lead to progressive enforcement.

Observations:
- General sanitation okay during inspection
- No signs of pests during inspection
- Professional pest control company is contracted to complete monthly inspections.
- Walk-in cooler at 3'C
- Chest freezers and display freezers at -20'C
- No food observed stored outside

Notes:
- Operator plans to start smoothie/fruit bar AND meat shop at the back of the facility. Plans have NOT yet been submitted by the operator or approved by the district EHO. Operator must submit plans to the health inspector PRIOR to operation of smoothie/fruit bar. Submission requirements discussed with staff during inspection.
