Observation (CORRECTED DURING INSPECTION): Walk in cooler was 12 C at time of inspection, food temperatures were 5-6 C.
Corrective Action(s): Adjusted temperature during inspection, temperature was found to be 4 C. Must ensure cooler temperature is 4 C or colder at all times.

311 - Premises not maintained as per approved plans [s. 6(1)(b)]
Observation: 1. Freezer was added without approval.
2. Floor around 3 compartment sink and dishwasher is lifting from water damage and no longer easily cleanable and may be a good place for pests to harbor.
Corrective Action(s): 1. Ensure to advise health department if new equipment is going to be installed.
Remove Styrofoam and cardboard base from freezer.
2. Repair floor in kitchen so it is smooth, easily cleanable and not lifting.

NOTE: observed customer entering kitchen to use washroom, customers are not permitted in the back kitchen area.
