Observation: Mouse observed caught in tin cat trap in the dry storage room. Mouse droppings observed in the dry storage area, outside managers office, hot water tank/electrical room, and bar.
Corrective Action(s): Thoroughly clean and sanitize the above noted areas. Thoroughly clean underneath and behind all equipment and all hard to reach areas. Contact pest control services and have more traps setup. Correct immediately. Followup inspection required.

Observation (CORRECTED DURING INSPECTION): Back door to premise propped open. Screen door is in place but mesh material is damaged potentially allowing the entrance of pests.
Corrective Action(s): Screen door not of adequate design as it has gap at the top to accomodate the self closing mechanism for the back door. Ensure the back door to the premise is kept closed. Open only briefly for when receiving supplies or taking out the garbage.
