Observation: Plastic movable cart noted next to elevator and large plastic bread box with lid used as food prep surfaces
Corrective Action(s): Ensure all surfaces in the premises are smooth, durable, easy to clean and used as intended.
-The cart and bread box are not suitable or permanent food prep surfaces. These areas should be replaced with proper prep tables and used as intended. Carts are used to transport food products and supplied. Change practice and designate space for all food prep.

Observation: 1. Plastic movable cart (in front of back standing freezer) noted with deep melted gouges in surface with food debris trapped
2. Plastic lids for bulk dry food bins are cracked - bin by elevator and bin by standing back freezer.
Corrective Action(s): 1.Ensure all carts are smooth, durable and easy to clean. The damaged cart is difficult to clean and sanitize and must be replaced.
Correction date: 3 days
2. Ensure all lids and other containers are in good condition and not damaged - concerns about broken pieces of plastic could become a physical hazard if those piece fall into food. Operator has ordered new bins.

Corrective Action(s): A) Ensure all holes/gaps in walls are sealed after plumbing work is completed to prevent possible pest entry and allow for proper cleaning.
*A concern with pest control to not note this in their pest control report under recommendations for the operator.
Correction date: 1 month

B) Ensure walk in freezer flooring is refinished to protect the floor and allow for proper cleaning.
Have a plan within 6 months when flooring can be refinished. The flooring should be completed within 1 year.

No signs of pest activity noted. Pest control provided by Green Valley - report (July 24, 2019) reviewed - no pest concerns noted
