Observation: Cockroach observed under the pop dispenser and mouse droppings build-up under pop dispenser and on floor next to safe.
Corrective Action(s): Premise sanitation is to improve immediately to eliminate food sources for pests and remove contamination from surfaces as a result of pest activity.

Observation: Food debris build-up observed at the followin locations: lower shelving / floor under pop dispenser, beverage cooler around and under, around and under ice machine, lower storage cabinets below front rice warmer, on floor under large front prep. cooler unit, on floor under deep fryer and stove, on floor between pop syrup and safe, and in right corner beside front cooking area.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.
Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine) after cleaning.
