Observation (CORRECTED DURING INSPECTION): Two of three hand washing stations were missing paper towels.
Corrective Action(s): All hand washing stations must be properly supplied (with hot and cold running water, liquid soap, and paper towels) at all times.
Staff stated one hand washing station is not used and the other paper towel dispenser was not working.
Roll of paper towel supplied at hand wash stations at time of inspection.

Observation: Operator has not paid the outstanding permit fee of $150. Current decal for operating permit has expired.
Corrective Action(s): Make payment by end of the day TODAY to avoid closure of facility. Follow payment instructions previously emailed to you.

Observation: Pizza slices hot held at temperatures below 60C, however are not held past 30 minutes (at time of inspection, temperatures ranged from 30C - 45C). No proper time-tracking system or logging is in place.
Corrective Action(s): Ensure the pizza slices currently in the hot-holding area are sold within the next 30 minutes (facility has been open for less than 1 hour). Create a time-tracking log for your hot-held pizza slices. Going forward, if there is no evidence of proper time-tracking for pizza slices, all slices WILL BE DISCARDED.

Comments

- One hand sink (next to three-compartment sink) was properly supplied with hot and cold running water, liquid soap, and paper towels in dispensers. *ALL hand washing stations must be properly supplied. See violation codes 401 and 308 above.
- Quats sanitizer solution available at 200 ppm
- Three-compartment sink available for manual warewashing
- Walk-in cooler, pizza prep cooler, and upright cooler measured at 4C or colder
- Walk-in freezer and undercounter freezer measured at -18C or colder.
- General sanitation satisfactory
- Dry storage and chemical storage areas satisfactory.
- No signs of pest activity noted at time of inspection.

**For pizza hot-holding: Ensure to have a time-tracking system in place. The health inspector should be able to review logs/records of pizza slices being hot-held and discarded. If logs are not available for future inspections, pizza slices WILL BE DISCARDED. See violation code 212 above.

**Outstanding permit fee of $150 must be paid by end of day today to avoid facility closure. See violation code 103 above.
