Observation: REPEAT: Tofu observed stored in metal bowl on top of inserts in top well of sandwich cooler. Internal temperature of Tofu observed at 21.5 C.
Corrective Action(s): Tofu was discarded. To ensure foods are maintained at 4 C or below, food its to be stored inside inserts of sandwich cooler well to allow adequate chilling of the product. Food stored on top of inserts in cooler well will not receive adequate refrigeration.

Observation (CORRECTED DURING INSPECTION): Stored clean hand held can opener observed with long-standing food debris on the blade.
Corrective Action(s): Can opener was placed in two compartment sink to be washed. Ensure all food contact surfaces are washed, rinsed, and sanitized after each use to prevent cross contamination.

Observation (CORRECTED DURING INSPECTION): Staff member observed not washing their hands after touching face mask, touching face, and after touching personal cell phone.
Corrective Action(s): Staff member was instructed to wash their hands. Staff then was educated on the importance of hand washing. To prevent the spread of disease, ensure hands are washed:
1. Before and after touching your face, mask, nose, eyes, or mouth
2. After using the toilet
3. Before and after preparing food
4. After eating food
5. After touching personal objects such as cell phones

Observation (CORRECTED DURING INSPECTION): 1. Clean pots and pans placed on floor under the grill.
2. Cutting board stored between counter and top of reach in cooler in front area.
3. A tool (hammer) observed stored with three can openers on preparation shelf.
4. Food stored uncovered in stand-up fridge in kitchen area
Corrective Action(s): To protect food and food contact surfaces from cross contamination:
1. Ensure all food and food contact surfaces are stored at least 6 inches from the floor. Pots were removed from the floor and placed in two compartment sink to be washed.
2. Ensure all food contact surfaces are stored in a sanitary manner. Cutting board was removed and placed in two compartment sink to be washed.
3. All tools and supplies are to be stored in a separate location from food. Hammer was removed and placed separate from food contact surfaces.
4. Ensure all food, not in the process of cooling, is covered in refrigeration units.

Observation: 1. Back door to kitchen observed open with a gap larger than 1/4 inch observed under screen door.
2. REPEAT: Accumulation of grease debris observed outside premises on sidewalk by back door.
Corrective Action(s): 1. To prevent the entry of pest, repair/replace the screen door or provide seal under screen door.
2. To prevent the attraction of pests, remove all grease from exhaust unit and side walk.

Observation: 1. Gaskets observed with build-up of food debris in stand-up fridge, reach-in coolers, and stand-up freezer in kitchen area.
2. Stand up fridge door in kitchen area observed with long-standing debris
3. Repeat: Outdoor exhaust unit observed with a build-up of grease.
4. Food debris observed behind sandwich cooler and shelving in kitchen area.
5. Build-up of grease observed behind deep frier in kitchen area.
6. Long standing debris observed accumulating around the corners of the grease trap.
Corrective Action(s): To maintain the premises in sanitary condition and prevent the attraction of pests:
1. Clean all gaskets, handles, and doors to refrigeration units regularly.
2. Remove all grease from floors and exhaust units.
3. Clean and remove all food debris and long standing debris from floors.

Observation: Cutting board observed in poor condition with deep gashes in its surface.
Corrective Action(s): To ensure all food contact surfaces are durable, smooth, and easy to clean, replace/repair cutting board.

- Low temperature dishwasher achieving 100 ppm at plate level
- No signs of pests
- Chlorine sanitizer throughout preparation areas observed at 200 ppm
- Chemicals observed stored separately from food
- Raw food stored below all ready to eat foods in refrigeration units

Corrections Needed:
- Place single use paper towels in a dispenser unit by hand sink to keep paper towels easily accessible.
