Scoops stored in ingredient bins. Ingredients can potentially become contaminated by staff when reaching in for the scoop. Dirty scoop handles can also contaminate ingredients.

All staff needs to be educated/trained in the importance of keeping ingredients clean.

Raw meats stored beside and above other foods in walk-in cooler. Potential contamination if raw meat juices drip onto other foods.

Rice and other dry foods stored in original paper bag. Products are in boiler room under pipes.

3. "Stalagmite" formed from frozen blood forming in 2 door freezer.
