Observation: 1.Observed ice box with potentially hazardous food products inside which did not have sufficient ice to hold food at 4 C or lower
2. Main cooler for storage is not in good working order; food products are kept above 4 C; measured food temp with probe thermometer 4.5-7.5 C;
Corrective Action(s): 1. Required operator to have ice replenished frequently so that container is fully submerged in ice. this is a repeated violation and if continued, this practice will have to be prohibited.
2. Required operator to move all potentially hazardous items to a working cooler and call technitian for repair immediately. Operator is to keep temperature to ensure that it is being monitored.

Corrective Action(s): Required operator to remove bubble wrap and duct tape, clean and sanitize the surface thoroughly. All surfaces must be durable, non-absorbant and easy to clean.
