Facility was serviced by a pest control operator on Nov 12, 2017 and report states low activity. No recommendations provided for operator. Corrective Action(s): Have pest control determine potential entry point for pests and seal off all potential entry points. Remove dead rodent front tin cat and sanitize surrounding areas. -Follow-up inspection to November 27, 2017. -Facility has been serviced by a pest control operator on Nov 12, 2017. Report indicates low activity and no other information. Operator needs to determine where the pests are coming from, and seal off any potential entry points. -General cleaning has been carried out in the walk-in cooler. Ensure shelving units are cleaned and sanitized on a regular basis and maintained in a clean and sanitary condition. -Pizza prep cooler door has been repaired. -Please contact the inspector if you have any questions or concerns.
