Observation (CORRECTED DURING INSPECTION): Main hand washing station beside cooking lines was blocked by a trolly and had utensils in the sink.
Corrective Action(s): Required operator to keep the area cleared of obstructions and have the hand washing sinks available for hand washing at all times.

Observation: Observed the following:
1. Found two dead small mice caught in a sticky trap beside the dishwasher
2. Observed droppings throughout the facility but no droppings were found near food prep area.
Corrective Action(s): Required operator to remove the carcass and clean up all areas of the restaurant. Monitoring of pest activity must be done each day to ensure infestation can be removed or prevented.

Observation: Observed the following concerns:
1. Back screen door was broken near the bottom that may allow pests to enter the premises
2. Hard to reach areas under and behind equipment had accumulation of debris
3. Rubbles, sand and holes in wall noted under dishwashing area.
Corrective Action(s): Required operator to do the following:
1. Fix the screen door to insure that no pests can enter the premises
2. Remove all equipment and clean the areas under and behind equipment, especially where droppings are found
3. Clean the area under dishwashing pit and block the hole in the wall.
4. Contact pest management to strengthen their approach to manage pest infestation, if active.
