Corrective action: discard the melted butter Stored in microwave and never Store melted butter in room temperature over night.

Observation:
1. Sanitizer bucket is not being used and wet wipping rags stored near hand sink was at 0ppm QUATS.
2. All QUATS Solution were expired on 2020/11/15
3. All dough making utensils and equipment are only being washed once per day after closing.
Corrective actions:
1. Always Store wet used rags Should be Stored in Sanitizer Solution to prevent microbial Contamination.
2. Replace all expired QUATS Solution
3. Ensure all utensils used are being washed and Sanitized at least once every 4 hours.

liquid hand soap was empty and unable to pump. Operator refilled liquid hand soap and fixed the pump on site.
Corrective action: Ensure hand sink is always supplied with hand soap to ensure proper hand washing.

Observation:
1) Scoops are being Stored inside the dry storage items including flours, Spices and Sugar with handles touching the food. A porpously broken Scoop was being used to scoop out flour. broken plastic scoop used is a physical hazard when the broken plastic pieces entered food.
Corrective action: Do Not Store scoops directly into the dry Storage food items and replace the broken plastic Scoop.

1) flies noted around the counter area
2) Rodent droppings noted under counter near the dough mixer, in corner area of the dish washing room, near mop sink and near drink coolers.
3) A dead mice was noted in the pest trap box near microwave area.
Corrective action:
1) place mechanical pest Control device near the Counter area to control flies infestation.
2) clean and disinfect areas where rodent droppings were noted with 100ppm chlorine (1 tablespoon bleach with 1 litre water). Increase frequency of pest control service.
