Observation (CORRECTED DURING INSPECTION): Back door was open, but a screen door was closed. The grates of the screen door and the top of the door has large openings at can allow for flies or other pests to enter the facility. Additionally, the floor sweep has gaps large enough to allow pests to enter the facility.
Corrective Action(s): Back door was closed by the operator at the time of inspection.
If operator wants to keep the back door open, a sufficient screen door must be installed that can mitigate the entrance of pests.

Observation: Ventilation hood grates above the cookline had built up oil and grease.
Corrective Action(s): Ensure that the grates are adequately cleaned and sanitized. Increase frequency of cleaning ventilation grates as required.

- Thawing procedures observed, water is changed every 30 minutes
- Handwashing station supplied with hot and cold running water, liquid soap, and paper towel
- Low temperature dishwasher available for mechanical warewashing, sanitizer concentration registered at 50 ppm chlorine on plate surface during final rinse cycle
- Two compartment sink available with drain plugs
- Food contact surface sanitizer concentration registered at 100 ppm chlorine
- Overall sanitation is satisfactory (see exception code 306)
- No signs of obvious pest activity
- Operator indicated licensed pest control services are in place
