Observation (CORRECTED DURING INSPECTION): The following potentially hazardous foods stored in the sandwich cooler were discarded due to time and temperature abuse (internal temperature logged):
- 2x 1/2 containers of green sauce (9.2 - 10.3C)
- 1/2 and full container of yogurt (9.8 - 10.8C)
- 1/6 container of tomato sauce (9.5C)
- ~500 g of cottage cheese (8.6C)
- 2x 1/2 jugs of milk (10.3-10.5C).

Observation: Active cockroach infestation observed throughout the facility. Number of live adult cockroaches and nymphs observed on each sticky traps. No routine pest control service in place.
Corrective Action(s): Routine weekly pest control service to be in place effective immediately. The frequency to be set weekly until directed otherwise.

Observation: Observed many attractants throughout the facility. For example, observed number of empty drink containers with leaked sticky residue on the floor by the hall way, food debris build up under the cook line, etc.
Corrective Action(s): The following actions to be taken immediately:
- Improve overall sanitation (especially hard to reach areas).
- Remove standing water or any liquid on the floor.
- Remove source of standing water/liquid (e.g., empty bottles).
- Remove any items that are stored directly on the floor. Provide >15cm elevation off the floor.

Observation: The sandwich cooler in the middle of the kitchen's ambient air temperature was measured at 10C.
Corrective Action(s): Do not store any potentially hazardous foods in this cooler. Service or adjust the cooler so that it can reach 4C or less.

Routine inspection conducted:
Hand washing station is fully equipped with hot and cold running water, liquid soap and paper towels.
Coolers (one except in the violation section) measured at 4C or less.
Freezers at adequate temperature.
Low temperature dishwasher's sanitizing cycle was measured at 100 ppm chlorine at the plate level.
Food contact surfaces sanitizer in a spray bottle was measured at 100 ppm chlorine.

Follow up inspection to be conducted in 2 weeks.
