Observation:
1. All parts of both dough mixers maintained in an unsanitary condition.
2. Meat slicer has accumulation of food debris in hard to reach areas.
3. Masher/cutter on wall has accumulation of food debris stuck on the slicers.
Corrective Action(s):
Thoroughly clean and sanitize all equipment listed above. Ensure to follow the manufacturers specifications and your sanitation plan for Clean in Place equipment.

Observation:
Permit to Operate is posted in the food preparation area.

Observation:
1. Dry flour stored directly in a garbage bag.
2. Scoop for flour stored directly in flour.
3. Plastic grocery bags used to store food products (meatballs and shredded cheese) in the stand-up freezer.
4. Food products, such as seasoning, flour and salt, stored directly on the floor in the back storage room.
Corrective Action(s):
1. Discard flour stored in a garbage bag. Ensure only food grade containers are used to store food products.
2. Place scoop in a designated container so the handle does not contaminate the flour.
3. Use food grade containers/bags to store food products. Plastic grocery bags are not food grade.
4. Ensure all food products are stored at least 15cm off the floor to prevent pests from getting into the food easily.

Observation:
Mice droppings found throughout the premises, especially:
- Behind all chest freezers
- Underneath sinks
- Dry storage rooms
- Back storage room

Person in charge informed that the facility does have a pest issue and pest control comes in regularly.
Corrective Action(s):
- Clean droppings in all areas listed above.
- Sanitize all food contact surfaces.
- Contact pest control for more frequent visits.
- Ensure all points of potential entry for mice are sealed.

Observation:
Back door was open at the time of inspection, however the screen door was closed. The screen door has large gaps between the bottom of the door and the floor tile. In addition, there are holes in the mesh screen.
Corrective Action(s):
Repair/replace the mesh screen and ensure there are no gaps at the bottom of the door to allow pests from entering.

General sanitation of premises not satisfactory, especially in the following areas:
- Shelves in walk-in cooler
- Underneath and behind all equipment
- Underneath dry storage shelves

Clean all the areas listed above. Ensure to review your sanitation plan for daily, weekly and monthly cleaning.

Observation:
Sandwich prep cooler temperature measured at 6.8 degrees Celsius. Non-potentially hazardous foods stored in the cooler at the time of inspection.
Corrective Action(s):
Repair prep cooler to ensure a temperature of 4 degrees or below is maintained. Do not store any potentially hazardous foods in this cooler until receiving approval from the Environmental Health Officer.

Two bleach sanitizer spray bottles were not labeled.

Label the spray bottles as "sanitizer" or "bleach" to prevent a chemical contamination.

Observation:
Two orchid plants stored on top of dishwasher near 3-compartment sink. Person in charge informed that they had no where else to place these plants.

Remove plants from food preparation area.

Comments

Routine Inspection:
Hand washing station fully equipped with liquid soap, paper towels and hot/cold running water.
Pizza makeline, walk-in cooler and beverages cooler maintained at or below 4 degrees Celsius.
Stand-up and chest freezers maintained below -18 degrees Celsius.
Hot holding unit turned off at the time of inspection. Ensure hot holding temperature reaches at least 60 degrees Celsius or above before pizzas are stored in the unit.
Concentration of bleach sanitizer in spray bottles = 200 ppm.

NOTE:
High temperature dishwasher is not used in this food premises as the 3-compartment sink is approved for manual warewashing. Either remove or block off dishwasher with tape to ensure it is not used as the dishwasher has not been serviced to reach the adequate sanitizing level.
Long chest freezer near stand-up freezer is not in use.
Personal food and cell phone was stored on top of meat slicer. Ensure equipment is used only for food preparation and not for storage of personal items.

COVID-19 safety measures in place, including:
COVID-19 safety plan in place.
Employee screening in place for symptoms.
Hand sanitizer available upon entry.
Physical barrier present between staff and customer at cash register.
High-touch surfaces are disinfected using Lysol wipes.
Maximum occupancy limit posted upon entry.
