Observation (CORRECTED DURING INSPECTION): Cooked potatoes, cooked rice, portioned rice dishes are noted in facility. Cooked rice (34.5oC) and portioned rice (35.6oC) are in the cooling process but saran wrap and lid are used to cover the foods. Chef indiated cooked potatoes (measured at 34oC) are just cooked.
Corrective Action(s): Foods must be properly cooled: 60oC to 20oC in 2 hours (at room temperature, no cover, small portion); and 20oC to 4oC in 4 hours in coolers.

Observation: Large amount of food products noted storing on ground, in kitchen and in walk-in cooler.
Corrective Action(s): Foods must be stored at least 6 inches above ground, to facilitate cleaning and for pest monitoring.

Observation: Decent amount of dead cockraches is noted on floor at cooking line (between undercounter freezer and preparation cooler). One live cockroach is noted in that area. Area under deep fryers and stove has too much grease to determine cockroach presence. At least one live cockroach is also noted in broiler room. Last pest control visit was 3 months ago.
Corrective Action(s): Contact a pest control company immediately for pest situations in facility.

In order to re-open:
1. Pest control company must visit at least once and work on the cockroach issue in facility
2. Extensive thorough cleaning must be done for the whole facility. Accumulation of filth and grease must be cleared.
