Corrective Action(s): Ensure prior to leaving for the night all food contact surfaces are properly washed and sanitized. As facility has severe rodent infestation any food debris must be cleaned before leaving for the night.

Observation: Dead mouse observed on glue board.
Rodent droppings observed throughout including in storage areas, food preparation areas, on dough mixer, under microwaves, on shop vac, behind chest freezer, under pop bottles, on cardboard boxes, etc.
Nesting materials (fiberglass insulation) found in storage room in corner.
Foot prints found on pizza equipment.

Corrective Action(s): Pest Technician servicing facility once per month. Discuss with pest technician.
Locate additional holes (two found surrounding plumbing of hand sink in front, one found next to nesting materials in storage room next to mop sink) and seal with non-chewable materials.

Observation: Holes found in drywall in various areas.
Food debris found throughout (behind chest freezer, in pizza prep area, etc.)

Corrective Action(s): All holes must be sealed with non-chewable materials. Have Pest technician inspect facility for holes. Obtain flashlight and look for additional holes. Inspect up in ceiling next to hot water tank.
Food debris MUST be removed from facility at end of night. Do not leave any food for rodents as this is an attractant.

Observation: Rodent droppings found throughout.
Facility is not maintained in usual high standard of cleanliness - food debris, food splatter, dirty equipment, cutting boards not cleaned from previous night but food preparation occurring.

Corrective Action(s): Facility is to be completely cleaned and sanitized throughout. All food contact surfaces, floor, closets, food storage areas, under equipment, under pop bottles, in cupboards, inside containers storing equipment, food splatter on walls, etc. must be cleaned and sanitized.

CLOSURE ORDER ISSUED:

REASON FOR CLOSURE ORDER: Violation of Food Premises Regulation Section 26 Every operator of food premises must ensure that the premises are (a) free of pests, (b) free of conditions that lead to the harbouring or breeding of pests, and (c) protected against the entrance of pests.

ITEMS TO BE CORRECTED:

1) Clean and sanitize all areas affected by rodent droppings. Remove all rodent droppings/feces from facility. All floors, shelves, equipment, utensils, dishes contaminated by rodent droppings must be cleaned and sanitized to remove rodent droppings. Remove all items from shelves and clean and sanitize. Move all items off floor to sanitize floor properly. Discard all contaminated food and single use utensils and dishes. Sanitize all tops of containers including canned food, bucket lids, etc. All surfaces (including floors, shelving units, buckets and counters) must be sanitized with 200ppm Chlorine or run through dishwasher
2) Fill any holes with non-chewable (i.e. metal or wood) materials. Seal exterior doors. Inspect remainder of facility for holes and seal
3) Cease food production IMMEDIATELY.
4) All items are to be stored up off floor 6 inches. Remove clutter. Organize. Discard garbage. This includes cupboards
5) Do not open facility without approval from Fraser Health Authority. Leave Order posted. Do not move, alter, cover, destroy Closure Order once posted. Contact Fraser Health Authority for opening inspection of facility
