Observation: Bucket of mandolins found to be improperly cleaned and put away as clean.
Wall mounted potato cutter found to be in unsanitary condition.
Cockroaches found on shelves where food equipment is stored.
Cockroaches found in cooler unit seals.

Correction: Clean and sanitize all areas affected by cockroaches.
Use 200ppm Chlorine.

- Facility has been thoroughly cleaned and sanitized
- Facility has been treated for cockroaches previous night for emergency treatment

ITEMS TO BE CORRECTED:
- Additional cleaning is required - some areas with large amounts of grease observed by EHO including grill area (legs of units and grease pile under grill)
- Continue to treat for cockroaches. Every two weeks as Pest Control Technician recommends
- More thorough and routine cleaning required. Facility needs more cleaning to ensure infestation is controlled. Cockroaches will not be attracted to bait if food debris is present in facility.
