Introduction

This is a unscheduled routine inspection to assess compliance with the Community Care and Assisted Living Act (C.C.A.L.A.), the Residential Care Regulation (R.C.R.) and the relevant Director of Licensing Standards of Practice (D.L.S.P.). Evidence for this report was based on the Licensing Officer's observations, review of the facility records, and information provided by the facility staff at the time of the inspection.

The following areas were reviewed:

As part of this routine inspection a facility risk assessment tool is completed and a copy is provided. The risk assessment includes non-compliance identified during the routine inspection, and a 3 year historical review of the facility's compliance and operation.

Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
POLICIES AND PROCEDURES: 33230 - RCR s.85(1)(b) - A licensee must do all of the following: (b) review and, if necessary, revise the policies and procedures at least once each year.
Observation: The policies and procedures required as per section 85 of the residential care regulation have not been reviewed and/or revised. The Manager will ensure this takes place for the entire building for the hospice and long term care components.
Corrective Action(s): Please ensure the policies and procedures required by section 85 of the residential care regulation are reviewed and/or revised once a year and that this is documented so it is evident.
Date to be Corrected: Please provide a written response to this item by the response timeline noted in this report.

CARE AND/OR SUPERVISION: 34360 - RCR s.63(3)(c)(iii) - A licensee must ensure that meals are provided (c) by ongoing room tray service, if (iii) approved by the person in care's medical practitioner or nurse practitioner.
Observation (CORRECTED DURING INSPECTION): A review of the persons in care charts indicated the medical practitioner had not been approved (signed off) for on-going tray service.
Corrective Action(s): Please ensure the medical practitioner or nurse practitioner approve on-going tray service and that this is reassessed every 30 days thereafter for example by a Dietitian.
Date to be Corrected: This was correct at the time of the routine inspection.

HYGIENE AND COMMUNICABLE DISEASE: 35020 - RCR s.49(1) - A licensee must require all persons admitted to a community care facility to comply with the Province's immunization and tuberculosis control programs.
Observation: For the hospice program, immunizations and tuberculosis clearance is not on the persons in care files. The writer emailed the Tuberculsosis and Immunization Guideline for persons in care on May 28, 2021 to the Manager and the Hospice Team Lead.
Corrective Action(s): Please ensure this is implemented immediately.

RECORDS AND REPORTING: 39320 - RCR s.83(4)(a) - Subject to subsection (5), a licensee, other than a licensee who provides a type of care described as Hospice, must (a) ensure that each person in care is weighed at least once each month.
Observation: For the long term care floors a random review of the persons in care charts indicated that some weights were not documented on a monthly basis (Specifics provided during the routine inspection).
Corrective Action(s): Please ensure that each person in care is weighed at least once each month. If there is a reason a weight is not taken, please document this (e.g., weight scale is functioning, etc).

RECORDS AND REPORTING: 39480 - RCR s.87(d) - A licensee must keep a record of the following matters respecting food services: (d) food services and nutrition care education and training programs attended by food services employees.
Observation: Currently there is no formal education around food services and nutritioncare provided for food service employees, and there is no documentation around this as well. The writer e-mailed to the Manager on May 28, 2021 sample education schedules to guide the management at the site to develop their own schedule(s).
Corrective Action(s): Please ensure a record of the following matters respecting food services is kept:
- food services and nutrition care education and training programs attended by food services employees.

Comments

General care requirements:
Regarding residential care regulation section 56 (1) - Identification of persons in care off-site this was discussed in detail. Please ensure even for a temporary outing by a person in care with their family for example that there is written documentation indicating the person in care's name, community care facility's name, and emergency contact information.
Persons in care charts:
For the hospice program a random review of the persons in care charts indicated the admission diagnoses is not consistent from one document to the next. Recommendation to ensure the admission diagnoses/information is consistent from one document to the next.
Staff checklist:
The writer e-mailed on May 28, 2021 to the Manager what is required to be on staff personnel files, and a sample checklist used for owned and operated Fraser Health facilities. The minimum that must be on-site for the hospice and long term care components is a staff checklist.
Policies and Procedures:
In discussion with the Hospice Lead Staff, it is recommended that the residential care regulation section 48 around addressing concerns with the Patient Care Quality Office and/or Medical Health Officer main switchboard number is noted only (Licensing Officer delegated on behalf of the Medical Health Officer) is added to the admission book/binder provided on admission so this process if formalized.
The Manager is in the process of ensuring fire drills are completed as required. Please let the writer know the plan to address this moving forward.
Nutrition and Food Services:
Nutrition and Food Services audits are being completed by the Food Services Manager, and Clinical Dietitian. Please forward the most recent nutrition care plan audit to the writer by e-mail.
Due to the Covid-19 pandemic, the findings were reviewed and discussed with the Manager at the time of the inspection. This inspection report, and risk assessment was written off-site and then emailed on June 11, 2021 to the Manager for review and to finalize the report and risk assessment once they were in agreement to the wording. As a result of the pandemic, signature for the Manager is not included. If there are further questions related to this routine inspection, please contact your Licensing Officer.
