Observation: Food items were observed to be:
1) stored on the ground in the kitchen area, as well as the walk in cooler
2) uncovered and unprotected against contamination
3) stored in unapproved containers
Corrective Action(s): Ensure that all food items are:
1) stored at least 6 inches off of the ground
2) covered with food grade material, such as plastic wrap, aluminum foil, or the appropriate lid
3) All food items must be stored in food grade material
To be corrected by: As soon as possible

Observation: Build up of grease, dirt and debris throughout the facility, specifically the:
- storage area
- walk in cooler
- hard to reach places such as behind or underneath equipment
Corrective Action(s): Routinely clean the facility, specifically the mentioned areas on a routine basis to ensure there is no build up of grease, dirt, and debris.

Observation: Ceiling tiles were missing in some sections in the kitchen area as well as some light fixture covers.
Corrective Action(s): Ensure to replace the ceiling tiles and light covers.
