Observation: Sanitizer concentration tested at approx 100 to 150ppm in the sanitizing sink. Air bubbles were observed entering the dispensing tube where the zip tie connector was located. Once tube was primed to eliminate any air in the tubing, the sink refilled and 3 pumps of quat sanitizer added, the concentration was tested at 200-300ppm.
Corrective Action(s): Due to no documentation for daily testing of the concentration of quat sanitizer, all utensils will need to be resanitized.

*Monitoring logs are not being maintained for the sanitizer concentration at the 3 compartment sink. Document daily that the staff have verified using test strips that 200ppm quat is present in the sanitizing sink.
***Ensure staff are properly documenting in the daily hourly log when the pizza slices have been placed in front holding unit ie fill out the documentation immediately after the pizza has been placed in the holding unit. As per your corporate food safety plan, the pizza slices are to be immediately discarded after 1 hour if not sold.
