Observation (CORRECTED DURING INSPECTION): QUATs sanitizer bottles (x2) measured at <100 ppm QUATs concentration with test strips. Staff prepared fresh QUATs sanitizer which measured at 200 ppm QUATs concentration during inspection.
Corrective Action(s): Prepare QUATs sanitizer daily at a concentration of 200 ppm for adequate sanitation of food contact surfaces and equipment (or as per approved sanitation plan). Verify sanitizer strength with available QUATs test strips at least once per day.

Observation (CORRECTED DURING INSPECTION): Hand washing station in staff washroom is not supplied with single-use paper towel or other single-use drying method. Staff placed paper towel at the dispenser during inspection.
Corrective Action(s): Maintain all designated hand washing stations with hot and cold running water, liquid soap and paper towels at all times to promote proper, frequent hand washing by food handlers and to prevent contamination of food.

209 - Food not protected from contamination [s. 12(a)]
Observation:
1. Multiple food items (curries, peeled onions) observed uncovered in the walk-in cooler.

2. Open can of tomato paste observed stored in the walk-in cooler. Operator voluntarily discarded the can of tomato paste during inspection.

3. 2 boxes of chicken observed stored directly on the floor next the receiving door during inspection. Staff placed the chicken on the walk-in cooler shelf during inspection.
Corrective Action(s):
1. Cover all foods in the walk-in cooler with a food grade cover (i.e. plastic wrap or food container lid). Keep all food covered to prevent potential contamination.

2. Opened cans of food must be used immediately. Any remainder food from the can must be discarded or transferred to a food grade container to be stored in the cooler.

3. Do not store any food or containers of food directly on the floor. All food must be stored at least 6 inches off the floor to prevent contamination.

Observation (CORRECTED DURING INSPECTION): Package of gravy sauce observed stored in a container at room temperature. Surface temperature of gravy measured at 0'C and observed still frozen. Staff placed the gravy sauce package in a container of cold water during inspection.
Corrective Action(s): Do not thaw potentially hazardous foods at room temperature to prevent time-temperature abuse and growth of bacteria and/or toxins. All potentially hazardous foods must be thawed using one of the following approved methods:
- At 4'C or lower (under refrigeration over night)
- In the original packaging in a cold water bath, changing the cold water every 20 minutes
- In the original packaging under cold running water

304 - Premises not free of pests [s. 26(a)]
Observation:
1. Multiple drain flies (>40) observed in the dishwashing area during inspection. Operator contacted the pest control company during inspection.

2. Rodent droppings observed on the floor in the equipment storage area next to the dough mixer.
Corrective Action(s):
1. Submit the last 2 months pest reports to the health inspector for review. Contact pest control company to visit the premise, assess the drain fly problem and apply treatment as necessary.

2. Remove pest droppings wearing appropriate PPE. Clean and sanitize the aforementioned area with bleach sanitizer at a concentration of 5000 ppm.

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation:
1. Back door and screen door observed wide open at the start of the inspection. Staff closed both doors during inspection.

2. Dirty mop water observed stored in the mop bucket at the back storage area.

3. Back storage area observed with multiple items on the floor. The floor is observed with clutter. .
Corrective Action(s):
1. The back door and screen door must remain CLOSED at all times to prevent pest (i.e. rodent and fly) entry on site. Ensure to use the screen door when needed.

2. Dump mop water in the appropriate designated area immediately (As per approved sanitation plan). DO NOT store dirty mop water as this can attract pests to breed/harbour on site.

3. Remove clutter from the floors in the storage area to facilitate frequent, proper cleaning of all floors, shelves and equipment. Clutter attracts pests such as rodents to breed/harbour on site.

Observation: General sanitation of premise requires improvement. The following areas are observed with food debris, grease or mold/mildew build-up:
- The wall behind the hand washing station in the kitchen
- The wall behind the preparation table in the kitchen
- All floors throughout the premise (including but not limited to the kitchen, back storage, underneath the 3-compartment sink, underneath all storage racks, behind and around all cooking equipment, in the staff washroom)
- Handles of all cooler doors
Corrective Action(s): Degrease, deep clean, and sanitize all aforementioned areas to remove food debris, grease, and or mold/mildew. Conduct regular cleaning of the premise (as per approved sanitation plan) to maintain the premise in a sanitary manner.

Observation (CORRECTED DURING INSPECTION): Two sanitizer spray bottles observed stored unlabelled. Staff labelled the bottles during inspection.
Corrective Action(s): Store all chemicals labelled to prevent chemical contamination of food. Ensure food contact surfaces sanitizers are always labelled to promote adequate, frequent sanitizng of food contact surfaces (As per approved sanitation plan).

- The designated hand washing station in the kitchen MUST NOT be used for dumping food. Designated hand washing stations are for handwashing ONLY
- Keep all sandwich cooler top covers closed when not in use to prevent temperature fluctuations
- Temperature logs must be maintained for all coolers and freezers
