Observation (CORRECTED DURING INSPECTION): Front lower cooler unit noted at 15 C ambient temp and 12 C probe thermometer yogurt and 4L milk jug.
Corrective Action(s): Ensure all PHFs (dairy products) maintained at or below 4 C. Discard all PHFs maintained above 4 C for over 4 hours.

Observation (CORRECTED DURING INSPECTION): Sanitizer bottle noted at <100 ppm QUATS in labelled spray bottle in back storage room. Unlabelled sanitizer mixing jug noted <200 ppm QUATS. No test strips available.
Corrective Action(s): Ensure sanitizer bottles are labelled and maintained at 200 ppm QUATS. Test strips must be available to test concentration of manual mixed solutions. Replenish sanitizer bottles, label and get test strips.

Observation: 1.Mouse droppings noted behind and under sink in back storage room.
2.Living cockroach trapped on sticky trap behind standing freezer in kitchen.
Corrective Action(s): Ensure all mouse droppings are cleaned up. Pest control report (Sept 27, 2018) reviewed and no note of status of pest activity. August 2018 report noted no pest activity inside. Ensure pest control technician is doing a thorough inspection and treatment of premises for both mice and cockroaches. Pest control onsite monthly. Increase pest control service for every 2 weeks until pest activity decreases. Continue clean up and monitoring of pest activity.

Corrective Action(s): Ensure coolers are maintained at or below 4 C. Do not store PHFs in this cooler until it is functioning properly. Monitor unit once repaired.

Observation: 1. Ice build up around walk in freezer - door cannot be closed
2. Ice build up on pipes around the top of walk in cooler
3. Wood cover over grease trap has gaps around the edges and does not close completely - smells of grease trap smelt above and round the cover.
Corrective Action(s): Ensure the above ice is melted to allow the walk in units to function as intended.
The cover over the grease trap must able to completely seal the above hatch to the grease trap - replace cover with tight fitting cover.
