Observation: - Mold/mildew noted on interior of ice machine. Manager reported ice machine is cleaned annually.
Corrective Action(s): - Empty ice machine and clean and sanitize interior per manufacturer's instructions
- Increase cleaning frequency of ice machine so that it is maintained in a sanitary condition.

Observation (CORRECTED DURING INSPECTION): Pies in walk in cooler were not covered
Corrective Action(s): Cover pies with food grade wrap or place in container with lid. Ensure all foods are covered/protected from contamination.

304 - Premises not free of pests [s. 26(a)]
Observation: - Mouse droppings noted on floor along edges of walls and between equipment (ice machine, stand mixer) in kitchen and on the floor under the shelving units in the dry storage room, particularly in the corner where the multi-catch trap is located.
- Chewed materials appear to be present behind ice machine and in corner of dry storage room.
- Mouse droppings were noted at past routine inspection and the issue appears to have worsened; however, limited to floor area. No damage to food products/packaging noted at time of the inspection.
Corrective Action(s): - Per code 306, move all moveable equipment and shelving and thoroughly clean floor to remove all droppings and disinfect floor in those areas. Continue to monitor for any further rodent activity (droppings, chewed materials) and if noted, clean up immediately. Implement monitoring and cleaning plan.
- Pest control contract is in place with twice monthly service for the exterior and interior of the building including the kitchen. Have Pest Control Techinician identify any entrance points or openings within kitchen. Block or cover any entrance points or openings to prevent access.

Observation: - Food debris, grime and mouse droppings noted on hard to reach areas of the floor such as between and behind equipment such as the ice maching and under shelving in dry storage room.
Corrective Action(s): - Move all moveable equipment and shelving units and thoroughly clean hard to reach areas of the floor to remove all food debris, grime and mouse droppings.

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation: - Sanitizer dispenser at 3 compartment sink was not working properly. When exterior button was pushed, solution would not dispense; however dispenser could dispense solution if opened and internal button was pushed
- Sanitizer dispenser near entrance to kitchen was dispensing quats sanitizer at 400ppm rather than at required level (for sanitizing food contact surfaces) of 200ppm
Corrective Action(s): - Repair sanitizer dispensers identified above so that they are in good working order and dispensing sanitizer solution at required concentration. If sanitizer dispenser near entrance to kitchen is used in the interim, dilute solution with water to 200ppm using test strips to check.
Note: Dispensers at warewashing sinks were dispensing sanitizer solution at required concentration (200ppm).

Observation: - Shatterguard missing from one light fixture in dry storage room.
- Shatterguard in walk-in freezer is not affixed fully to light fixture and is held in place with tape
- Lower portion of wall at entrance to kitchen near handsink/coffee preparation is damaged/deteriorating so that surface is no longer smooth, washable and in good repair.
Corrective Action(s): - Repair/replace shatterguards on light fixtures.
- Repair portion of wall or affix suitable coving so that wall finish is smooth, washable and in good repair.

Comments

- Handsinks were supplied with hot and cold running water liquid soap and paper towel
- Walk-in cooler was at 3C
- Walk-in freezer was -10C
- Cooler and freezer temperatures are monitored and recorded twice daily.
- Hot holding of potentially hazardous foods in steam table was >60C. Scrambled eggs 65C, Gravy 69C
- High temperature dishwasher was operational and met sanitizing rinse temperature. Temperature at dish surface was 75.9C
- Dishwasher wash and rinse temperatures are monitored and recorded three times daily with external gauges. Thermal labels are also present and reported to be used weekly.
- Quats sanitizer present and in use for sanitizing food contact surfaces. Solution was at 400ppm quats initially; however diluted to 200ppm once staff were notified. Test strips present for checking concentration - ensure test strips are being used.
- Foods in dry storage room were stored on shelving (6 inches off of the floor) and dry goods stored in plastic containers.
- Raw meat products were stored beneath ready to eat items within and raw eggs were stored away from ready to eat items in the walk in cooler. Aside from pies noted in violation code 209, foods in walk-in cooler were covered.
