Observation (CORRECTED DURING INSPECTION): Cut vegetables and sashimi fish stored at 6-7C instead of 15C from last week.
Corrective Action(s): Although temperature has dropped it is still not at 4C. Transferred all fish to working cooler. Discard all cut vegetables over 2 hour mark.

Observation (CORRECTED DURING INSPECTION): A black garbage bag used to store sashimi fish in the cooler.
Corrective Action(s): Do not use garbage bag to store food, use food grade bags only.

Observation: Walls on the mop sink room have not been cleaned as there were dark marks still.
Corrective Action(s): Please have the walls properly cleaned as this room has high chance to harbour fruit flies.

Observation: No hot water dispensed from kitchen handsink.

Observation: Kitchen handsink faucet cannot dispense hot water when turned. Sushi bar handsink faucet and sink itself is not secured, causing some food debris to collect between sink and counter.

Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
Observation: Cut produce in the undercounter sushi cooler was measured at 15C; operator turned down temperature at time of inspection. He increased the temperature as it was freezing food. Five boxes of cream cheese stored in cooler in the back storage area was measured at 15C. Opened tubs of commercially prepared mayonnaise are stored at room temperature dispite label indicating refrigeration required after opening.
Correction: Undercounter cooler was measured at 7C at end of inspection and continuing to drop. Five boxes of cream cheese were discarded. Always store cold potentially hazardous food at 4C or colder. Always store food in refrigeration after opening as per label.
Comments

Ice machine cleaned and ice made appears clear without any debris. Please ensure to clean the ice machine more frequently from now on.
Hot water restored at kitchen handsink.
Faucet secured at sushi bar handsink.
Bucket of water removed from tempura station.
Floor tiles replaced in kitchen.
Spills and food debris removed on sushi bar wooden shelf, floors of sushi bar, kitchen floors and dry storage room next to dishwasher. Overall sanitation level improved.
Fruit flies continued to be observed in dry storage room next to dishwasher, please keep sanitation level up here.
Bleach sanitizer available in kitchen and sushi bar at 100ppm.
No food observed to be stored on the floor or uncovered at time of inspection.

Note: Gaskets ordered to be replaced for all coolers where gaskets are loose or broken. Expected to be delivered and installed in about 3 weeks. Thermometers in place in refrigeration units.
Cream cheese cooler in the back also waiting for new gasket installment. Avocado and whole vegetables stored here, all potentially hazardous food stored in two door cooler next to this.
Sushi bar cooler reduced amount of food stored inside, mainly cut vegetables. Fish is stored in under-counter cooler further inside where temperature is at 4C.
