Observation (CORRECTED DURING INSPECTION): Ice machine had slime and grime.
Corrective Action(s): Empty ice machine and clean and sanitize thoroughly.

Observation: Walk-in cooler and freezer floor have debris build up. Underneath cooking and prep tables have grease and grime accumulation. Line freezer (storage of fries, etc) requires thorough cleaning. Staff chang room/ washrooms was not tidy.
Corrective Action(s): Thoroughly clean the back kitchen area especially the areas/equipment noted in the observation section. Also tidy up the staff washrooms and change room areas.

Food items received from approved sources. Tags available for shell fish products.
Food items are stored off the floor (at least 6").
Refrigeration units registered at or below 4 deg C.
Cold foods (front of the line) were stored at or below 4 d eg C.
Frozen foods in freezer registered at or below -18 deg C.
All raw animal products were stored all together in separate shelves.
Food contact surfaces cleaned and sanitized (200 ppm quats).
Dishwasher had >100 ppm chlorine residual.
Hand wash stations adequately maintained.
Dining room area was clean and rest rooms clean and stocked with liquid soap and paper towels.
Bar area: hand sink provided with liquid soap and paper towels.
Sanitizer in spray bottle at 200 ppm. Glass ware washer had 12.5 ppm Iodine. Provide test strip for Iodine.
No signs of pests noted at time of visit. Pest control in place.
Staff had clean uniforms and hair restraint.
