Observation (CORRECTED DURING INSPECTION): **This is a repeated violation** A pair scissor was noted stored wedged in between a cooler and wall.
Corrective Action(s): **Issue corrected during the inspection** Operator removed the scissor and rewash and sanitize the scissor before the next use. Do not store any food contact items/ utensils in an unsanitary manner, in a location that is hard to clean and sanitize.

Observation (CORRECTED DURING INSPECTION): Missing paper towel by the server handwash station.
Corrective Action(s): **Issue corrected at the time of inspection** Operator placed a roll of paper towel by the front handwash station. Please ensure that all handwash stations must be equipped with liquid soap and paper towel at all time to allow staff to perform proper handwashing when required.

Observation: A bowl was noted used as a dispenser and sitting within a bulk container of broth.
Corrective Action(s): Remove the bowl and use a proper dispenser with handle and store the dispenser in a way where the handle does not come into contact with the food to prevent potential cross-contamination.

Observation: Overall cleanliness of the facility and number of mouse droppings noted have definitely improved. The following minor cleanliness issues were still noted during the inspection:
- Small amount of mouse droppings noted below the rack holding dry food and pots.
- Black buildup noted in the interior door of the dishmachine.
- Food soil/ buildup noted on the handle of the food prep handwash station - hot water faucet handle.
Corrective Action(s): Clean and sanitize the aforementioned areas and maintain in a sanitary condition at all time.

Comments

Routine inspection conducted today:
- Handwash stations, accept for the one cited in the above violations, were accessible, equipped with hot and cold running water, liquid soap and paper towel.
- All refrigeration units were at 4.0C or below.
- All freezer units were at -18C or below.
- Food noted stored protected and in the proper storage hierarchy.
- Rice was holding at 80.0C in the hot hold unit.
- 2 spray bottles of bleach sanitizer were available (one in the server area and one in the food prep area). Both detected with 200 PPM chlorine residual.
- Drain plugs available for the 2-compartment sink.
- High temperature dish machine was able to reach 186F at the rinse temperature gauge and 78.5C at the plate level during the rinse cycle.
