HAZARD RATING FOR YOUR FACILITY: Moderate (16-29) Total Score = 24
Critical Hazards: Total Number: 1
211 - Semi critical items not adequately cleaned and disinfected
Observation (CORRECTED DURING INSPECTION): High level disinfectant is being diluted by adding water (and is supposed to be used undiluted) and has expired (way beyond expiry date on label). Semi critical items are not being disinfected with high level disinfectant.
Corrective Action(s): Bleach being used for laundry can be used for disinfecting semi critical items (cuticle nippers, pushers, nail clippers, drill bits if metal and re-used, tweezers, etc.). Use 5000 ppm chlorine as discussed by diluting 1/4 cup (62.5 mL) of domestic bleach in 2 cups of water (1/2 L or 500mL of water). Make the chlorine solution new every morning and discard at the end of each day. If using high level CS20, buy a new bottle and use before it expires so check expiry dates.
Violation Score: 15

Non-Critical Hazards: Total Number: 2
220 - Non critical single use items are not discarded after use
Observation (CORRECTED DURING INSPECTION): Drill bit / disposable sanding bands are not being discarded after use but are being left on the drill bit.
Corrective Action(s): Discard all paper or disposable sanding bands and any single use items immediately after use (as soon as client leaves) so they will not be re-used.
Violation Score: 9

331 - Premises not maintained in clean, sanitary or pest free condition
Observation: Room used for waxing and eyelash tinting/extensions does not appear to be clean. Sink in this room requires cleaning.
Corrective Action(s): Clean and disinfect sink in treatment room. Clean all shelves and surfaces and keep the nail and esthetics salon clean by cleaning daily.

Follow Up to "Critical" Violations Noted on Previous Inspections (if applicable): No corrections entered

Nails:
- Critical items - credo/pedicure blade is single use only (not re-used) and is disposed of into a sharps container available near clean up sink.
- Semi critical items (cuticle nippers, pushers, nail clippers, etc.) require high level disinfection - none available - see above.
- Single use (disposable items) - foam toe separators are single use, single use foam emery boards - see above re drill bit sanding bands.
- Foot bath - a disposable plastic liner is used (good) and after liner is discarded, foot bath is cleaned and disinfected prior to using a new liner (use 20 mL chlorine bleach in 1 liter water).
- Foot bath impeller is not used.
- Callus removal - using a pumice stone or porous callus pad (single client - clients take home) - *** Note that these should be single use (discarded after each use) as they are porous and cannot be cleaned and disinfected.
- No ingrown nail treatment (sends to physician).
Esthetics:
- Eyelash and brow tints - labelled professional use only - good.
- Smooth stones - clean and disinfect (using bleach) after each use.
Waxing:
- Single use waxing strips and wooden spatulas.
- Wax appears clean today.
- No removal of ingrown hairs.
Eyelash Extensions:
- Training - eyelash extensions can only be provided by person with training (operator AN).
- Eyelash glue appears to have proper Health Canada labelling.
- Tweezers - use the high level disinfectant as described above as these are used around the eyes and are semi critical items.
- Sink in room has hot/cold water, liquid soap, and single use hand drying towels.
- Room cleaning - see above.
General:
- Hand washing sink and clean up sink in main nail salon area.
- Esthetics room has hand sink.
- Hand sinks have hot/cold water, liquid soap, and single use hand drying towels.
- Washroom - for clients and staff.
- On site laundry - using bleach. Use hot dry for drying linens and towels.
- Esthetics room has a carpet for standing on - recommend a washable mat rather than a carpet.
- Cleaning - see above.
- Chemicals - check expiry dates. TB disinfectant and CS20 disinfectants have expired - do not use.
- VIM bottle labelled bleach - does not have bleach in it.
*** Follow directions on the label for the safe and proper use of all chemicals. Label should accurately show what is in the bottle. Use chemicals in well ventilated areas.
- Note: The floor is laminate flooring and is becoming worn. Maintain surfaces in good condition - ok for now.
