Observation (CORRECTED DURING INSPECTION): One insert of rice located in one of the hot holding units was measured at an internal food temperature of 55 degrees C. This is a repeat violation. Further repeat violations may result in enforcement action.
Corrective Action(s): Rice was reheated to an internal food temperature of at least 74 degrees C at the time of inspection and hot holding unit temperature was increased. Ensure that all staff are following all procedures in the food safety plan. Discussed at time of inspection.

Observation (CORRECTED DURING INSPECTION): Two of three bottles labeled sanitizer measured at almost no detectable quaternary ammonium compounds (quats).
Corrective Action(s): Bottles emptied and refilled at dispensing unit, then re-tested at 200 ppm quats. Ensure that all staff are following procedures outlined in sanitation plan.

Food safety and sanitation plan on site at time of inspection. Ensure that this is available to all staff and that staff are trained in food safety and sanitation procedures. Relevant sections were reviewed with management at time of inspection.
Quaternary ammonium compounds dispensing unit may potentially be dispensing sanitizer at variable concentrations. Ensure that (1) dispensing unit is being used correctly, and (2) dispensing unit is in good working order. Unit was dispensing correct concentration at time of inspection when larger volumes were dispensed from the unit.
Follow up to be conducted within one month.
