PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11010 - CCLR s.13(1) - A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation: In the outdoor playground:
-The gate leading down to the garage had a large gap underneath where a child could crawl through - this leads to concrete steps which increases the risk of injury. Repeat Contravention from inspection# SWAR-CJQR3R
-The gate at the bottom of the steps leading up to the deck and the gate at the top of the steps were both open and accessible to children while in care
Corrective Action(s): please ensure you provide a healthy and safe environment for children while in care.

RECORDS AND REPORTING: 19090 - CCLR s.57(2)(c) - A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation: Licensing reviewed the attendance record, only the record for the day was available for review. Licensee stated she had removed the previous months attendance and filed it away in her home. The attendance that was available for review indicated that 1 child was not signed in.
Corrective Action(s): Please ensure attendance record is available on site for review. Please ensure children are signed in/out on a daily basis.

(b) 2 files were missing the phone number of a medical practitioner
(c) 3 files were missing the name and phone number of an emergency contact
