Observation: There's some algae/mold growth along the pipes in the ice machine.
Correction: Remove all the ice. Clean the area thoroughly.
Comments

On Follow-Up:
Mold growth on pipes inside the ice machine has been cleaned. Clean the pipes regularly.
200ppm bleach solution is now being used to sanitize surfaces.
Cooler/freezer temperatures are now being recorded daily. ENSURE THE DISPLAY CASE IS ADDED TO THIS LIST ONCE IT'S BEEN FIXED.
The base of the tap at the 2-compartment sink is no longer loose.
Each of the staff members is in the process of getting their FS level 1 certification. One of the employees on duty had already taken the course and passed. They provided a letter as proof.

To be Corrected:
The display case had not been fixed. There were a few PHF's stored in the case for display purposes only. They are discarded at the end of the day. The rest of the PHF's were being stored in one of the coolers. Operator had ordered the wrong part. The new part that's required to fix the cooler is on order. Do not store any PHF's (except for the display items that are discarded at the end of the day) in the display cooler until it's fixed and can maintain a temperature of 4 degC or below.
