Observation: Rodent droppings noted throughout facility, particularly in the following areas:
-shelves under the mini deep fryer
-behind prep coolers
-on the bread cart
-behind the bread cart (under microwave)
-in the chemical storage area under the 2 comp sink drainboard
-on the dishwasher chemical cart
-shelf with mechanical slicer
-behind chest freezer
-under the grill station
-around be bar electrical extension cords
-in the bar printer overstock cabinet
-around the beer pitcher storage
-in the office beside the shelf with board games
-various shelves in the downstairs dry storage room and under them
this is not an exhaustive list
Corrective Action(s): Remove ALL equipment from the walls and conduct a deep clean of the facility. This must be done in the kitchen, bar, and downstairs storage room.

Observation: several issues noted:
-entry hole noted by wire/pipe into floor behind the bread storage (under microwave)
-entry hole noted by wire/pipe into floor under the potato press
-chewed up napkins noted behind prep cooler
-shredded insulation noted in basement storage area
-shredded insulation noted in corner of office (adjacent to shelf of games)
Corrective Action(s): Clean the shredded insulation and sanitize areas.
Fill the entry holes and pull out various equipment to identify any entry holes

=mini chest freezer (-18C) and walk-in freezer (-19C) measured < -18 degrees C
=Gravy hot holding (75C) measured > 60 degrees C
=Low temperature dishwasher measured >50 ppm chlorine at the plate surface after priming machine
=Glasswasher measured 25 ppm iodine at the dish surface (minimum 12.5ppm required for proper sanitizing
=General sanitation was satisfactory at the time of inspection
=General food storage practices satisfactory at the time of inspection. Foods properly stored off the floor and covered and raw meats stored below ready-to-eat foods.
=Ice machine maintained in a sanitary condition
=Permit posted in a conspicuous location

note: new pizza station added outside the kitchen (adjacent). Pizza oven, handwash station, and pizza prep cooler (2C) added. No objections with extension of kitchen however, future changes to the kitchen will require prior approval
