Observation (CORRECTED DURING INSPECTION): Meat slicer has dried meat debris noted around guard and sharpener.
Corrective Action(s): Ensure entire meat slicer is taken apart completely, cleaned and sanitized after each use. All parts including guard and sharpener that can be directly or indirectly become a food contact surface must be cleaned properly. Operator cleaned and sanitized slicer during inspection.

Observation: Bulk dry food bin with Rice and Sugar stored directly on the ground next to prep cooler.
Corrective Action(s): Ensure all food containers are stored at least 6 inch off the ground to protect food from contamination and allow for proper cleaning and pest monitoring. Operator will re-organize space to accommodate requirements.

Observation: Small gap between corner of door and frame at side door next to bar opening to parking lot.
Corrective Action(s): Ensure all gaps around/under doors opening to outside premise are securely sealed to prevent pest entry. Seal gaps are door.

Follow-up inspection:
-Significant improvement in sanitation - Kitchen and back storage space re-organized and cleaned under and around containers and equipment
-Low temp mechanical dishwasher repaired - noted 100 ppm chlorine at the plate (satisfactory)
-Sanitizer bucket in kitchen and bar with wiping cloths noted 100 ppm chlorine
-Sanitizer bottle noted 200 ppm QUATS with clean wiping cloths available
-Raw bloody meat moved to right cooler and produce and ready to eat food stored in middle cooler.
-Scooper in bar area stored in sanitary container outside jelly
-Ceiling tile re-installed in back bar storage room
-Single use disposable gloves have been discarded and not on counters to be re-used
-Inside cooler have been cleaned and re-organized. Food all covered with lids or plastic.
-Insert covers have been placed over openings (empty inserts) on the top of prep cooler in dining area. (Cooler maintaining <4 C)
