Pallet sitting on top of pallet with boxed/packaged products. Corrective Action(s): Ensure all pest infested/contaminated food sacks are discarded immediately. Contaminated food is not fit for human consumption. Discard all contaminated food /entire pallet. Violation Score: 5

Dead mouse found on 2 glue boards Corrective Action(s): - Remove droppings and clean up affected areas. - Rotate pallets - clean underneath. Ensure all areas of the warehouse are accessible for pest control service. - Continue to monitor rodent activity and report activity to pest control technicians. - Ensure loading doors are closed when not in use to prevent pest entry. Stacked wooden boards against walls at end of rows 2.Gap under double doors connecting unit to Everland unit next door 3. Spilt sesame seeds over lower pallet and floor in back of off walk in cooler (right side) Corrective Action(s): 1. Ensure all build up or wood and other materials are properly stacked and put away to avoid preventing pest monitoring and control. 2. Re-install door sweep under 2 double door connecting unit to Everland unit. Correction date: 1 month 3. Clean up all spilt food to avoid attracting pests as a possible food source.
