Observation (CORRECTED DURING INSPECTION): QUAT sanitizer is made available in a spray bottle. However, over 400 PPM QUAT detected.
Corrective Action(s): **Issue corrected during the inspection** Staff diluted the spray bottle and 400 PPM was detected after the dilution. Please use the wall mounted dispenser as the concentration is pre-set for use. When the concentration is too high, sanitizer will not completely evaporate when used on food contact surfaces and may be transferred to food causing a chemical hazard to consumers.

Observation: Live mouse noted in the mop room (tail sighted under the door of the mop room) during the inspection.
Corrective Action(s): Ensure the premises is kept clean and sanitize of debris and food soil at all time to eliminate any food source for pest.
Increase pest control visit to biweekly and continue working with the pest control company to deter pest issues noted at the facility.

Observation: 1. **Note: This is a repeated violation** No food-grade sanitizer or one that staff is aware is available in the front of house where food is being served.
2. **Note: This is a repeated violation** Dried food debris noted on the wall mounted potato peeler.
Correction: 1. Ensure to have a bottle of food-grade sanitizer spray bottle or container available anywhere food is prepared or served. Staff must know where this is located and use it as appropriate.
2. Clean and sanitize the potato peeler. Follow your sanitation plan and clean and sanitize after every use.
**Any future repeated violations WILL result in progressive enforcement actions, which may include Order, Ticket, Closure, Court, etc.**

Observation: 1. No hand soap was available by the handwash station in the front register area.
2. Paper towel were not loaded into the paper towel dispensers by the handwash station in the front register area and in the washroom.
Correction: 1. **Issue corrected at the time of inspection** Soap was replaced in the soap dispenser. Ensure handwash station must be stocked at all time with liquid soap and paper towel in the appropirate dispensers.
2. Load the paper towel in the dispensers to protect it from contamination before use.

Follow-up inspection conducted for report# KLAI-CULS72
- Hot hold unit for sausages and taquitos were holding food at above 60C.
- No unprotected food stored in the self-serve coffee area.
- Potato peeler now noted in a sanitary condition.
- Soap was noted available at the handwash station by the cash register. Paper towel was loaded into the dispenser.
- No personal items were noted stored with customers' items during the inspection.
- General cleanliness has noted improved but debris and food soil still noted under the shelves in the back storage area. Remember to clean and sanitize often due to the ongoing pest issue.

- No further follow-up required at this time.
