Observation: * Grime/debris found on shelving through out kitchen and sushi bar and storage areas; all food areas to be cleaned and maintained properly at all times

Observation: * Hand sink faucet in kitchen was broken and water access at this sink was difficult as faucet hard to use; operator to repair immediately

* Dishwasher final rinse was 50 ppm chlorine residual at plate surface(internally)
* Cold-holding units were 4'C/40'F or colder
* Freezer units were -18'C or colder
* Monitoring records were available but cleaning records must be updated as cleaning issues were noted through out (daily/weekly/monthly)
* Food was covered in storage areas
* Staff were wearing proper attire and hair protection
* Hand sinks had liquid soap and paper towels
* Note: hand sink faucet in kitchen was broken and requires repair immediately as it is difficult to use the faucet
* Note: cleaning to be done through out kitchen and storage areas
* Note: staff to wash hands properly with soap and then dry with paper towel(single use towels)
