Observation: Quat sanitizer in spray bottle at 100 ppm (the repackaging room).
Corrective Action(s): Immediately. Replace sanitizer. Quat must be at 200 ppm.

304 - Premises not free of pests [s. 26(a)]
Observation: Small amount of mice droppings found in the store (bulk bins, under pallets, and near the walk-in freezer wall).
Corrective Action(s): Please clean all mice droppings. Pests Service Provider available on site. Continue to monitor pests issue.

309 - Chemicals, cleansers, & similar agents stored or labeled improperly [s. 27]
Observation: Noted two spray containers with cleaning agents are not labeled at the time of inspection (Meat Department).
A container with blue liquid/equipment cleaning agent/ is stored in the upper middle shelf in the walk-in cooler.
Corrective Action(s): Immediately label all containers with cleaning agents. All chemicals must be labeled properly.
Immediately remove the container with blue liquid/equipment cleaning agent. (Corrected at the time of inspection). Ensure that all chemicals stored in dedicated area.

Comments

General sanitation is acceptable; however, some cleaning has to be conducted.
Hand sinks (packaging room and meat department) supplied with liquid soap and paper towel.
Quat sanitizer - available.
Wall-mounted Quat dispenser near three-compartment sink in the meat department dispense Quat at 200 ppm.
Spray sanitizer in the meat department at 200 ppm.
Freezers - satisfactory.
Coolers < 4.0 C.
Most of food items are stored of the floor, in original packages, covered, & pests proof containers with lids.
Only pre-package dry goods were for sale at the time of inspection.

Recommendations:
1. Replace broken thermometer in the display cooler (meat department).
2. Remove a bar soap from the hand washing sink (meat department)./Corrected at the time of inspection.
Note: The operator claims that the bar soap is for three-compartment sink cleaning. Please, follow your sanitation plan procedures.
3. Remove from the store display shelves all dented cans.
4. Bulk food sections remain closed (covered at the time of inspection). Ensure that all gravity bins as well as containers with scops properly cleaned and sanitized. All scoops must be stored safely.
Note: The Health Department will notify the operator when bulk bins can be in use.
5. Repackaging food products must be properly labeled; and invoices from supply companies must be kept.
6. Wall mounted Quat dispenser must be re-calibrated in the repackaging room. Please check Quat sanitizer (Expiration Date and concentration)/bit low.

Emphasize to the operator COVID-19 safety procedures in a food facility.
- The operator at the cash register isn't wearing PPE (surgical or procedure masks; a shield screen protector - unavailable).
