Observation (CORRECTED DURING INSPECTION): A small container of enoki mushrooms was observed on the counter at ambient temperature. Mushrooms at 17 degrees C; air temperature at approx. 25 degrees C.
It was stated that mushrooms were removed from the cooler one hour ago.
Corrective Action(s): Mushrooms placed into nearby under counter cooler. Ensure all potentially hazardous foods are stored at 4 degrees C or less in order to limit the growth of potential pathogens (e.g. bacteria) and the potential production of toxins.

401 - Adequate handwashing stations not available for employees [s. 21(4)]
Observation (CORRECTED DURING INSPECTION): Hand soap was not available at the front area hand washing station (it was located at the nearby 2-compartment prep sink instead).
No hand soap was available at the back area hand washing station.
Corrective Action(s): Liquid hand soap provided for both hand washing stations. Ensure all hand washing stations are unobstructed and properly supplied with hot and cold running water, liquid soap, and paper towels at all times to facilitate proper hand washing.

402 - Employee does not wash hands properly or at adequate frequency [s. 21(3)]
Observation (CORRECTED DURING INSPECTION): Staff member was observed washing gloved hands.
Corrective Action(s): Gloves discarded. Staff washed hands properly and donned a new pair of gloves.
Ensure proper glove use and hand washing are practiced by employees. Single use gloves are not intended to be washed; gloves must be discarded and proper hand washing done prior to putting on a new pair of gloves, whenever hand washing must be performed.

209 - Food not protected from contamination [s. 12(a)]
Observation (CORRECTED DURING INSPECTION): 1. Raw chicken in plastic containers and raw shell eggs observed to be stored above cabbage, lettuce, and sprouts in the walk-in cooler. Note: No signs of potential contamination. Sprouts and lettuce bagged.
2. Bleach water sanitizer in a spray bottle labeled "sani" was tested at well over 200 ppm chlorine bleach.
Corrective Action(s): 1. Raw animal products were placed on the lowest shelf of the walk-in cooler, away from and not above ready-to-eat foods. Ensure raw animal products are never stored above ready-to-eat foods to avoid potential contamination of these foods in case there is leakage.
2. Bleach water sanitizer was diluted to 200 ppm chlorine bleach. Ensure sanitizer is maintained at safe and effective concentrations for sanitizing food contact surfaces (e.g. 100 to 200 ppm chlorine bleach).

Observation: Mouse droppings observed in the following area:
On the floor along the wall:
- Behind and around the chest freezer located near the walk-in cooler
- On the left side of the dishwashing area
Corrective Action(s): Ensure the above-noted areas are thoroughly cleaned to remove rodent droppings. It is recommended to use 1 part bleach to 9 parts water for affected areas. Continue to monitor for new pest activity and clean/sanitize affected areas as needed. Date to be corrected by: Today.

309 - Chemicals, cleansers, & similar agents stored or labeled improperly [s. 27]
Observation (CORRECTED DURING INSPECTION): One spray bottle containing bleach water sanitizer at 100 to 200 ppm chlorine was unlabeled.
Another spray bottle (located in the dish pit) contained mostly bleach, for cleaning purposes only in the dish pit--e.g. floors, sinks--and was unlabeled.
Corrective Action(s): Spray bottles were labeled at the time of inspection to identify their contents/purpose. Ensure all chemicals such as cleaners and sanitizers are properly labeled to help prevent potential misuse of these chemicals (and potential contamination of food/food contact surfaces).
