Observation: 1 .Bar - flooring under glass washer littered with beverage container caps and broken glass and accumulating grease, food debris and dust.
2. Dishwasher area and under/behind alll heavy equipment in the kitchen - grease and dust accumulating
3. Bar - black mold like growth and grease collecting on plastic beverage lines by the ice well
4. Coffee station - hand sink does not have a back splash; the wall behind the hand sink is damaged and unsanitary.
Corrective Action(s): 1 and 2 - clean the affected areas on or before Dec 10, 2016
3 - clean the beverage lines on or before Nov 24, 2016
4 - clean the wall behind sink on or before Nov 24, 2016. Provide an appropriate back splash on or before Jan 20, 2016

Observation: Marble food prep surface at the pizza station is cracked. Flour accumulation is noted along the crack.
