Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation: Sanitizer spray bottle is broken. The solution did not have any detectable sanitizer in it.
Correction: Purchase a new spray bottle and fill it with fresh sanitizer. Refresh regularly.

Equipment/facilities/hot & cold water for sanitary maintenance not adequate [s. 17(3); s. 4(1)(f)]
Observation: The wood chopping block is caked with debris, grease and has dents on it's surface.
Correction: Thoroughly clean the entire block of wood as discussed. If the block is observed in this condition again, you will be required to replace it with equipment that is non-absorbent, and easy to clean.

Adequate handwashing stations not available for employees [s. 21(4)]
Observation: It's obvious that the soap in the dispenser at the handsink in the back of house (BOH) is not being used. It's thick and has a gel consistency now.
Correction: Clean out the dispenser and put fresh soap in it. Chef must use soap for handwashing.
Comments

On Follow-Up:
Cleaning issues have been addressed.
Chopping block has been scrubbed clean and resurfaced to make it more even.
The chest freezer lid has been fixed.
A lamp and "push light" has been provided in the storage room upstairs.
A new sanitizer spray bottle has been purchased for the FOH.
Processed foods are no longer stored in cardboard boxes.
The wall mounted soap dispenser has been removed from the wall and replaced with a pump soap bottle. If at any inspection, it is noted that the pump soap bottle is not at the back hand sink, you will be required to replace the wall mounted soap dispenser.
