Observation: A) Water accumulating below sinks and ponding in around the chemical bottles. Appears to be a water leak.
B) Handsink beside prep cooler plugged, dirty and not usable.
C) Prep cooler not repaired, but instead is supplemented with ice.**
Corrective Action(s): A) Identify and eliminate water leak.
B) Repair and clean sink such that the water drains.
C) Repair prep cooler such that it does not require ice to supplement temperature control on food. DO NOT STORE ANY POTENTIALLY HAZARDOUS FOODS IN THIS UNIT UNTIL REPAIRED.

Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)] (Prep cooler observed at range between 8.5 C and 10 C. Left unit undisturbed for 15 minutes with motor engaged, but without any change in temperature. Also observed the thermometer in the unit was broken so no way to monitor unit effectively.)
Comments

Ceiling cleaned up and tiles replaced.
Floor below dish sink cleaned up (but water ponding/leak remains - see above for corrective action).
