304 - Premises not free of pests [s. 26(a)]
Observation: Evidence of mouse infestation (mice droppings and urine) noticed in the following areas of the kitchen ( repeat violation):
-NE corner of the receiving area near the loading door
-Underneath some dry storage shelving
-Behind the "Emergency Storage" water bottle pallet.
-Along the North wall in the spare cart and shelving rack storage area
-Near the entrance to the decommissioned walk-in cooler
-Along the West wall in the tray/portioning (cold room) room.
(Note: Pest monitoring logs from the pest control contractor indicated "no activity" on its recent inspection reports)
Pictures taken.
Corrective Action(s): -A special inspection must be conducted by the pest control contractor.
-Clean all affected areas throughly using the floor cleaner after removing all equipment, carts and shelves from the wall corners.
-Disinfect the affected floors and floor-wall junctions with bleach and hot water following the cleaning March 29, 2019).
-Wash and sanitize all utensils stored on the stainless steel shelf near decommissioned walk-in cooler (immediately).
- All baby food in sealed glass jars/cans must be washed and then stored in covered totes.
-Pest control operator must provide more mice traps throughout the kitchen especially in the dry storage and receiving areas
-Install weather-stripping underneath the receiving area loading doors to prevent rodent entry ( ASAP).

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: -Significant gaps noticed underneath the loading area doors as well as underneath the LMH main receiving doors near the mail room. Weather stripping found to be inadequate on the receiving door in Rosewood/Marwood ECU.
-A hole was noticed in the drywall behind water botttle pallet and the baseboard was found be to be ripped off.
-An open cut off pipe was noticed near the decommissioned walk-in cooler; this could lead to rodent harborage.
Corrective Action(s): All gaps underneath all of the above doors must be elliminated either by repairing or replacing the wearther stripping.
(Immediately). Doors must stay closed when not in use and not supervised by receiving staff.
-The open cut off floor pipe must be sealed completely ( By March 29th, 2019)

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: -Build-up of food debris and beverage spills noticed along the floor-wall junctions underneath the counters in portion prep area.
- Floor in the main walk-in cooler requires cleaning.
- Dried mildew noticed on the light fixture in walk-in cooler #1
Corrective Action(s): -Throughly clean the floors in all areas of the kitchen especially underneath and behind equipment ( March 29, 2019)
-Clean or replace the light fixture in walk-in cooler #1 ( April 20, 2019).
- A new cleaning checklist and audit mechanism must be develoepd for janitorial staff outlning proper cleaing procedures in hard-to-reach areas. Staff must be trained to identify signs of pest infestation. (April 20, 2019).

309 - Chemicals, cleansers, & similar agents stored or labeled improperly [s. 27]
Observation: -A spray bottle of Ostrosan sanitizer was found to be stored in a bottle labelled "Haddon Cleaner".
-Since there has been a change of chemical supplier to replace Ecolab products with Haddon's products, the sanitizer log sheet was not revised to reflect the accepable concentration for new sanitizer ( Ostrosan sanitizer). Ostro san sanitizer must be used at 200 ppm ( if used as a no-rinse sanitizer) as opposed to the previously used Oasis146 sanitizer which is acceptable between 200-400 ppm range.
-Ostro san (quats sanitizer) in the third sink tested 400 ppm (too high to be used as no-rinse sanitizer)
Corrective Action(s): -Chemicals such as cleaners and sanitizers in spray botles must be labelled accurately ( immediately).
-Revise the sanitizer log sheets throughout all kitchens and ECUs to reflect the new maximum level of quats santiizer ( 200 ppm)
-Adjust the Ostro san sanitizer level at the dispenser to 200 ppm ( immediately).

*Urgent attention is required to address the mice issue and preventing entry of pests by checking all exterior doors.

Main walk-in cooler < 4 degrees C.
Walk-in freezer below -18 C.
All other walk-in coolers were between 0-4 degrees C.
Sandwich preparation cooler < 4 degrees C.
Main dishwasher reached 77 degrees C during final rinse cycle.
Under-counter dishwasher: 76.9 C during final rinse.
Internal temperature of hot foods in Burlodge cart: 74 C.
Temperature logs are up to date.

Rosewood/Marwood ECU:
Walk-in cooler below 4 degrees C.
Internal cooking temperature of soups >90 C.
Temperature logs up to date.
