Observation: Potentially hazardous food items such as sticky rice cake with vegetables or meat, meat buns, seafood canned in oil/vinegar (with lid on jar that is popped up), bamboo wrap sticky rice, desserts, coconut desserts and other items obtained from "lady that walks in". Most items are unlabeled, half at room temperature, half stored in the fridge. Lady does not present her food safe, company label, unknown if operating out of house or other premise. Operator seems to think lady is operating out of her house.
This is the second or third occurance of such foods available for sale here.
Corrective Action(s): Card provided again for "lady".
Foods must be from approved sources, receipts must be kept as records and available for inspection.
items for sale must be made in an approved facility.
Discard items that have no label and are potentially hazardous.

Observation: Operator using a homemade sticky trap with sugar type gel, wrapped in plastic, in a container, sitting amongst food items for sale.
Corrective Action(s): This is not an acceptable form of pest control.
Using bait is not recommended.
Use acceptable form, such as one provided by pest control contractor, purchased sticky traps or light traps.
