Observation: Accumulation of debris and garbage noted under all shelving and equipment in front and in storage room.Also all equipment surfaces require cleaning/staff washroom and mop sink are in bad condition.
Corrective Action(s): Clean the whole facility prior to the follow up inspection. Increase the frequency of cleaning to maintain proper sanitation for this facility- provide a revised sanitation plan to reflect this change.
